The Center for Health Care Innovation (CHCI) creates new health delivery strategies for communities throughout CHI by:

• Using existing clinical solutions in new ways
• Introducing brand-new clinical care models

Accountable Care Capability

To provide optimal patient care in the future, CHI will build integrated care delivery systems. Person-centered, population-based care also will be key to our success under new risk-based payment models.

The CHI team working on our Accountable Care Capability focuses on:

• Care Delivery – Strengthening provider alignment, payer-provider collaboration, infrastructure and technology to enable improved care coordination and management to targeted performance levels
• Health Data Management – Develop requirements to direct CHI’s population health data management capability
• Care Management – Ensure seamless integration of care processes across caregivers and delivery sites
• Risk models – Develop modeling tools and implement risk arrangement to deliver value-based payment systems
